Preparing for your Appointment

There may be a fair amount of ground to cover at your first appointment with a nurse practitioner, so it is often helpful to prepare.

Consider the names and contact information of important people or places in your life that you would like to share with the nurse practitioner. This may include a medical provider, another therapist, a case manager or a sponsor. You might consider having our nurse practitioners communicate with them, so you might bring along contact information.

Perhaps there is some helpful documentation or records you can share. This might be related to past or previous care by another provider. If you take a medication, this might pertain to records of prescriptions.

Also consider other obligations and your personal schedule in setting a regular time with your nurse practitioner.

Lastly, it might be helpful to think of questions, concerns or goals you might have as a client at St. Paul’s Center.
